Abstract
There is no doubt that transition programming for students is needed and extremely important. There have been many great programs and initiatives to help support students. However, researchers have found that school districts can better support our students to enter the world as independent adults. There are things that we can start implementing, even at a young age, to better prepare our students for their futures. This project outlines what transition programming needs to include in order to prepare students for the future. It also provides additional resources to implement person-centered planning.
